Application Checklist

Dear Fellowship Candidate:

Thank you for your interest in our program. We participate in the ERAS program. Here is the information that we request. We will only accept applications that are submitted through ERAS. Applications with all supporting documents must be submitted by January 31, 2008.

• Universal application

• Curriculum Vitae including a personal statement

• Picture

• Copy of medical school transcript

• Three letters of recommendation

• Letter of recommendation from Program Director or Chief of Medicine

• ECFMG certificate

• USMLE scores (actual sheets) and a record of the number of times section was taken

• Copy of state license (if available)
